DELAYED CREDIT CARD PAYMENT AUTHORIZATION ADDENDUM
This Delayed Credit Card Payment Authorization Addendum (“Addendum”) is made as part of the Terms of Service and Terms and Conditions or the Master Service Agreement (“Agreement”) between Formstack LLC (“Formstack”) and ____________________ (“Customer”), collectively “the parties”, and is effective by the date of execution.
Customer has requested the ability to store full credit card data, including sensitive authentication data, for a period of time before authorization. Formstack has created such a service for delayed payment authorization.
If the customer using this delayed payment authorization service, the Customer agrees to immediately notify Formstack when the payment is authorized by using the “Reveal and Process” button provided. The customer further agrees that if the “Reveal and Process” button is not used within ninety (90) days, the credit card data, including sensitive authentication data, will be truncated to the last four (4) digits of the credit card and expiration date of the card.
